What is Extracorporeal Shockwave Therapy (ESWT)

Extracorporeal Shockwave Therapy (ESWT) is a non‑invasive treatment that delivers acoustic shockwaves to injured tissues to reduce pain and stimulate healing. At Two Rivers Integrative, our physicians offer both focused and radial ESWT as well as Electromagnetic Transduction Therapy (EMTT). These modalities target musculoskeletal conditions without surgery, injections, or long recovery times.

What ESWT does to the body

  • Stimulates tissue repair: mechanical energy from shockwaves activates cellular pathways that increase blood flow, growth factor release, and tissue regeneration.
  • Reduces pain: neuromodulatory effects decrease pain signaling and lower sensitivity in affected areas.
  • Breaks down chronically altered tissue: shockwaves can help remodel calcifications, scar tissue, and chronically degenerated tendon fibers.

What research says about ESWT

Clinical studies show ESWT can be effective for many chronic tendon and soft tissue conditions, improving pain, function, and patient satisfaction compared with placebo or continued conservative care. Evidence supports particular benefit for plantar fasciitis, Achilles tendinopathy, and certain shoulder and elbow tendinopathies. Outcomes depend on appropriate patient selection, protocol (focused vs radial vs EMTT), and treatment accuracy; our team will recommend the best approach for your specific condition utilizing all our tools.

What to expect from treatments — during and after

A typical treatment series will include eight EMTT treatments and four focused and/or radial shockwave treatments all given over a four week period.  There is minimal downtime required following treatments like there is following PRP treatments. In the 24-48 hours following treatments patients may want to ease into more strenuous activity, but otherwise can use the joint at normal capacity directly following treatments.

EMTT is generally well tolerated and painless and typically lasts 10-15 minutes per joint. 

During ESWT treatments most patients feel strong pulses, tapping, or pressure at the site being treated. 

After treatment

  • Immediate: some soreness, redness, or mild swelling can occur and usually resolves in a few days.
  • Activity: brief rest may be advised for 24–48 hours with gradual return to normal activities; your clinician will give specific guidance.
  • Onset of benefit: many patients notice gradual improvement over several weeks; tissue remodeling and functional gains may continue for months.
  • Follow‑up: your physician will monitor progress and adjust the plan as needed.

  • Insurance coverage for ESWT and EMTT is limited. Most patients should expect to pay cash.
  • Each treatment is $225. Most patients find lasting relief after 3-4 treatments.
